San Diego Unified has partnered with UC San Diego to provide FREE COVID-19 testing at every school campus for all of our students. Testing is an important part of our district’s plan to protect our community from the spread of coronavirus. 

Our school’s scheduled testing days are May 20th and June 3rd prior to the last day of school. 

Participating in this free COVID-19 test requires parent consent. If you would like your child to be tested, please follow these TWO steps. You only need to complete this process one time. 

Step 1. Select the LINK (QR code provided) for your child’s school and schedule an appointment. After you select a time, you can sign into your current UCSD MyChart account or you can check in as a Guest. If you do not have a UCSD MyChart account you can create an account. Please note: test results will be sent directly to the UCSD MyChart accounts.

Step 2. Complete TWO forms, either online or via paper copy: Consent Form and Release of Information (ROI).

  • ONLINE Forms: Fill out the forms electronically via DocuSign

  • PAPER Copy: Download the two forms below and return to the health office. You may pick up a paper copy from the school site.

After following these two steps, UCSD will automatically schedule follow up testing appointments every two weeks during our school’s assigned time: Thursday May 20 and June 3 from 10:00 am to 12:30 pm.
