Every Roosevelt IB Student should have a signed SDUSD “Universal Form” on file for the 2020-2021 Academic Year. A signed “Universal Form” acknowledges that you have taken the time to review the “Facts for Parents Guide,” which offers a comprehensive look at SDUSD policies and is a valuable source of information. Thank you for helping SDUSD provide Students with the best possible education at this challenging time.

Please print, complete and sign the “Universal Form.” To submit your form you may use any of the following methods by October 9th:
- Scan or take a photo (with your phone) of your completed form and email that photo to Ms. Barajas in the RMS School Office: cbarajas1@sandi.net. Please use Subject Line “Universal Form 2020-21.”
- Mail your form to Roosevelt IB Middle School: 3366 Park Blvd, San Diego, CA 92103
- Drop off your form at the School Office this week, Monday – Friday, 8AM – 12PM.
